Birth Registration / Certificates

Parents can now register their childiren using the Newborn Registration Service at www.serviceontario.ca/newborn which electronically registers the birth directly with Service Ontario's Office of the Registrar General. In addition to registering the birth of their child, this new service allows parents to submit electronic applications for a birth certificate and social insurance number at the same time. Parents are encouraged to use this “bundled service” as it is the quickest way to obtain the birth certificate for your child.
Paper forms can still be completed manually and forwarded to the Office of the Registrar General. Please remember to use only blue or black ink to complete the Statement of Live Birth. Should you make a mistake, simply stroke a line through the error. Both parents must initial the change. Do not use correction fluid or the form will be void. You can also include a completed Birth Certificate application and fee in the same envelope.
Travel Letters
Travel letters are no longer available from the Municipality. If parents are required to travel with their newborn and have not applied for a birth certificate at the time of registration, they are to contact the Office of the Registrar General at 1-800-461-2156 or visit www.serviceontario.ca.
Birth Certificate Application Forms
Applications for Birth Certificates can be obtained from the Municipality of Leamington or downloaded from the Provincial website at www.serviceontario.ca. For your convenience, you may access it by clicking on the attached Birth Certificate Application Form link.
The Province of Ontario now offers an online Birth Certificate application process and status update (subject to some conditions). It generally takes approximately 4 weeks to receive a Birth Certificate. Service Ontario also offers a premium online service, in order to obtain a birth certificate at a faster rate.

MARRIAGES LICENCES & CEREMONIES
MARRIAGE LICENCE REQUIREMENTS

FEE $130 (cash or debit – no personal cheques)
AGE
Applicants must be 18 years of age or over to obtain a licence; however, applicants
who are 16 or 17 years of age may obtain a licence if they comply with the provisions
of the Marriage Act respecting parental consent. The “Consent of Parent or Guardian
to Marriage” form must be completed and signed by the parents(s) or guardian(s) at
the Municipal Office. No person under the age of 16 years may be married.
ISSUING OF LICENCE
Both applicants must sign the Marriage Licence application form. A Marriage Licence can only be issued to one or both of the applicants. When issued to only one of the applicants, the required identification of the absent party must be produced.
IDENTIFICATION REQUIRED
Any two of the following current pieces provided at least one piece contains a current photo. This identification must be provided for both individuals.
* Birth Certificate * Government Issued Photo ID (i.e. Driver's Licence)
* Passport * Valid Immigration Status Papers (i.e. Record of Landing,
* Canadian Citizenship Permanent Residency Card, Work Visa)
* Change of Name Certificate
Note: Photocopies will not be accepted and documents in a language other than English or
French must be translated by a certified translator.
PUBLICATION OF BANNS
Please confirm with your clergyman the need to obtain a Marriage Licence.
DIVORCE OR ANNULMENT
If the marriage was dissolved or annulled in Canada, the original or court-certified copy of the final decree, or certificate of divorce dissolving or annulling the marriage must be produced for examination by the Marriage Licence Issuer. A copy can only be certified by the Court which granted the divorce. A photocopy that has not been certified by the court will not be accepted.
OUT OF COUNTRY DIVORCE
IF THE DIVORCE WAS GRANTED OUTSIDE OF CANADA, the applicant must obtain authorization from the Office of the Registrar General. Please consult the Clerk’s office or an Ontario lawyer for information regarding this procedure as it may take several weeks to meet these requirements. ADDRESS
If you live at a rural route number or have a box number as an address, we will require the 911 address
for the property.
LICENCE VALID
A Marriage Licence is valid anywhere in Ontario for a period of (3) months from the date of issuance.
OFFICE HOURS
Marriage Licences are issued at the Municipal Office during office hours from 8:30 am to 4:30 pm., Monday to Friday. Marriage Licences are not issued after 4:00 pm
CIVIL WEDDING CEREMONIES
During office hours - $240.00 + 13% HST = $271.20
During office hours (other than English) - $285.00 + 13% HST = $322.05
After office hours and/or offsite - $320.00 + 13% HST = $361.60
Mileage costs beyond Leamington boundaries - $0.52 per km will be charged

Marriage or Death Certificates
Marriage or Death Certificate application forms can be obtained from the Municipality of Leamington or by clicking on the attached Marriage Certificate form and Death Certificate form
NOTE***
The Province has now guaranteed that client's marriage or death certificate will be mailed to them within 15 business days or they will receive their money back. Delivery time (by regular mail) is extra as marriage and death certificates are not sent by courier. This service guarantee is available to anyone who applies for a marriage or death certificate online where the marriage or death took place and was registered in 1991 or after, and where the application meets the required terms and conditions.
