Birth Registrations & Deaths Certificates
Registering your Baby’s Birth
You may register your child’s birth online through ServiceOntario.
ServiceOntario provides a “bundled service option” where parents can register their baby and, at the same time, apply for the following:
- Birth Certificate
- Social Insurance Number
- Canada Child Benefits (including the Ontario Child Benefit)

Birth Certificates
New or replacement Ontario birth certificates are issued by ServiceOntario.
- Online Request for Birth Certificate
- PDF Request for Birth Certificate (must be mailed or faxed to ServiceOntario)
Death Certificates
When a death occurs in Ontario, the funeral director registers the death with the Municipal Office and the Municipality sends the documents to the Office of the Registrar General to be registered. Please note that this process may take up to 12 weeks. After this is completed, you may apply for a Death Certificate from ServiceOntario.
If applying online, the certified copy of death registration does not include medical cause of death.
A certified copy of death registration with cause of death information can only be obtained by applying by mail or in person.
